IMPORTANT HEALTH COVERAGE TAX DOCUMENTS INFORMATION

With the new reporting requirements for 1095-C distributions, employers are no longer required to mail 1095-C forms to employees. 
Everyone is able to access a copy  of their 1095-C by going to Employee Online from the District website under staff links,  just like you would for your W-2 or check stub. 

If you are no longer an employee of the District, a 1095-C was mailed to your last known address.

You are able to request your 1095-C form. Please submit a request by:

  • Emailing the Benefits Coordinator at [email protected]
  • Calling the Benefits Coordinator at (248) 435-8400 ext 1214

Once a request is received, we will provide your Form 1095-C within 30 days of your request.
