Effective January 1, 2015, the State of Michigan changed the way that nonmedical immunization waivers can be obtained. Any parent/guardian who wants to claim a non-medical waiver will need to receive education regarding the benefits of vaccinations and the risk of disease from a county health department before obtaining the certified non-medical waiver form through the local Health Department.

If your child has a medical reason for not receiving a vaccine, a physician must sign the State of Michigan Medical Contradiction form; this form is available at your doctor's office (not the county Health Department). The completed Immunization Waiver (medical or non-medical) is then turned into your child's school.
