Effective January 1, 2015, the State of Michigan changed the way that nonmedical immunization waivers can be obtained.  Any parent/guardian who wants to claim a nonmedical waiver will need to receive education regarding the benefits of vaccinations and the risk of disease from a county health department before obtaining the certified nonmedical waiver form through the local Health Department.  The new rule requires the use of the State of Michigan nonmedical waiver form dated January 1, 2015.

If your child has a medical reason for not receiving a vaccine, a physician must sign the State of Michigan Medical Contradiction form; this form is available at your doctor's office (not the county Health Department). The completed Immunization Waiver (medical or nonmedical) is then turned into your child's school.
